Black Bear North Basketball program, the organizers, volunteers and all facilities being rented by Black Bear North assume no responsibility for illness due to COVID 19 or any other illness or any injuries or death to a team member, player named above and or to any spectator. All rights and claims are waived by the undersigned parent and guardian. The undersigned parent or guardian gives approval for the participation of their child in all activities of the Black Bear North Basketball program during the current season and assume all risks and hazard incidental to the conduct of the activities and transportation to and from the activities.
